Insurance Information     We participate with the following health insurance companies: BC/BS BC/BS Preferred and Preferred Plus PPOM PHP Priority Health Physicians Care ASR Weyco Medicare     If your insurance is not listed above, please ask us about it.     At the time of your visit you are responsible for payment of medical services not covered including deductible amounts. We will verify your eligibility at the first visit and attempt to check on your scope of benefits. We ask that you also be aware of your insurance requirements and what they will and will not cover. It is your responsibility to understand your coverage, but we will do all we can to make the process go more smoothly.     Many insurances require a referral from your primary care physician. Please be sure to obtain the appropriate referral for each visit prior to your arrival to avoid delays and/or denial of services. Having a referral in our office the day of your appointment will help us concentrate on your health concern.     If you anticipate a balance not covered by your insurance, please call us so we can set up a mutually agreeable payment plan. We want you to get well and are willing to work hard with you to make that possible.
